remove department_id in medical camp (camp master table) and also removed in the medical camp schedule page (ui)¶

add department_id in appointment table and also add in the medical camp appointment page¶

if user select the type as ortho or opthel we need to chose that same type in department and disable that input¶

if user select the type as general enable department field¶

add the village_id in fileupload service¶
